SRCS experts have advanced knowledge in navigating and utilizing community-based, senior-supportive services such as the Area Agency on Aging (AAA). We offer assistance locating resources within the AAA and creating a careplan called an Individual Care
Solution (ICS). The ICS is designed to meet the needs of the care recipient and carepartner. A fifteen minute phone consultation must be completed prior to the ICS.

    medical appointments

2. Can help you identify weak areas in your current careplan
 

3. Can alleviate carepartner stress and burnout
 

4. Can help you identify and strengthen your care village
